EXAMPLES OF OUR SPECIAL CHILDREN
Angel
(not her real name) is a five-year old exceptional little
girl who was born prematurely at 26 weeks gestation. She came
into this world with multiple respiratory complications. She
required a tracheostomy and mechanical ventilation. APC works
along with the hospital discharge planning team to facilitate
transition to her home. Nurses reinforced the teaching the
parents received at the hospital along with direct care. Angel
received Ventilator Care, Tracheostomy Care, Gastrostomy Tube
Care, Breathing Therapy, and observation for seizure activities.
Angel’s family received information and support from
our Medical Social Worker who is a vital part of our Welcome
Home Team.
Hope (not her real name) is a two-year-old beautiful baby
girl born with Encephalopathy. She has difficulty clearing
and maintaining her airway. APC nurses are able to assist
with treatment of upper respiratory infections and prevent
frequent hospitalizations. Providing assessment and reinforcement
of her parent's knowledge and skills allows this special
needs child to remain at home and a part of her family's
day-to-day life.
